On Leave

If a student needs a leave of absence due to illness or childbearing/adoption, please fill in the following form and submit it to the graduate school.

If a student wishes to take off a semester that is not due to illness (leave of absence), please fill in the follwing form and submit it to the graduate school.

The student may return the following semester without having to submit any other forms – may simply register and resume.
